Why Your BMI Number Might Be Misleading You

BMI measures a ratio, not your health. The difference matters more than most BMI calculators let on.

Body Mass Index — weight in kilograms divided by height in metres squared — was developed in the 1830s by the Belgian mathematician Adolphe Quetelet, not as a health metric at all, but as a way to describe the statistical distribution of body proportions across a population for census-style analysis. It was adopted as a clinical screening tool more than a century later, largely because it is cheap: it requires only a scale and a tape measure, unlike body fat measurement methods (DEXA scans, hydrostatic weighing, skinfold calipers) that need equipment or training. That origin as a population-statistics tool, repurposed for individual screening, explains most of its well-documented limitations.

What BMI actually measures

BMI is a ratio of total mass to height. It cannot distinguish between the mass of muscle, bone, organs, and fat — it only sees the total. Two people at an identical height and weight, and therefore an identical BMI, can have dramatically different body compositions and dramatically different health profiles.

Where it breaks down

  • Muscular individuals. Athletes and anyone with significant muscle mass are routinely classified as "overweight" or "obese" by BMI despite low body fat percentages, because muscle is denser than fat and BMI cannot tell the two apart.
  • Older adults. Age-related muscle loss (sarcopenia) means an older adult can have a "normal" BMI while carrying a much higher proportion of body fat than a younger person at the same BMI — the reverse problem from the athlete case.
  • Body-fat distribution. BMI says nothing about where fat is carried. Visceral fat around the abdomen carries a different (generally higher) cardiometabolic risk profile than fat carried elsewhere, at an identical BMI and even an identical total body fat percentage.
  • Population and ethnicity differences. Research since the 1990s has found that the health-risk relationship between BMI and outcomes like diabetes risk shifts across ethnic populations — several health bodies now recommend lower BMI thresholds for some Asian populations, reflecting a different risk curve at the same BMI value.
  • Pregnancy, and unusually short or tall individuals. The formula's simple height-squared scaling does not hold up well outside the population it was originally fit to.

Why it is still used

None of this makes BMI useless. As a free, near-instant, no-equipment screening tool applied across large populations, it correlates reasonably well with health risk on average, which is exactly what it was never intended but happens to be reasonably good at: population-level surveillance rather than individual diagnosis. Public health bodies use it because it is cheap to collect at scale, not because it is the most accurate individual measure available.

What to use instead — or alongside it

If you want a fuller picture of body composition than BMI alone provides, waist circumference (or waist-to-height ratio) adds information about fat distribution that BMI misses entirely, at a similarly low cost. The practical takeaway: treat a BMI number as one input among several, not a verdict. If your BMI classification doesn't match how you look, feel, or perform physically — especially if you train seriously or are older — that mismatch is expected, not a sign the calculator is broken.
